Dr. Leo Lombardo is a physician in active practice and board certified in both Anesthesiology and Pain Medicine. He holds B.A. and B.S. degrees from Stanford University and received his medical degree from the University of California, San Diego. While attending medical school, Dr. Lombardo was awarded the School of Medicine Dean’s Scholarship, the Pharmacology Education and Research Foundation Award and was a member of the Gold Humanism Honor Society. In addition to completing his internship in Internal Medicine and residency in Anesthesiology, he pursued fellowship training in Pain Medicine. Dr. Lombardo specializes in interventional pain management and multidisciplinary approaches to chronic pain. He maintains clear and active licenses in the states of California, Texas and Colorado. He is experienced in Disability, Worker’s Compensation, Automotive cases and Group Health. In addition, he is Colorado Worker’s Compensation (Level 1) accredited. He is also bilingual (English and Japanese).
Dr. Lundquist provides medical oversight over Dane Street’s Disability program. He is board certified by both the American Board of Internal Medicine and the American Board of Preventive Medicine in occupational medicine. He completed his internal medicine residency and occupational and environmental medicine fellowship at Yale University, where he also earned a Master of Public Health. Dr. Lundquist received his medical degree from Stony Brook University Medical Center School of Medicine.
Dr. Lundquist is currently a member of both the American College of Occupational and Environmental Medicine and the New England College of Occupational and Environmental Medicine. He recently served as the President of the New England College of Occupational and Environmental Medicine from 2018-2020. He is specifically interested in public health, employer wellness programs and the role of insurance in health care. Dr. Lundquist resides in Connecticut and interacts closely with our Physician Panel and Quality Assurance staff in Palm Beach Gardens, FL.
Dr. Williams currently serves as the Psychology Program Director and Director of the Psychological Clinic at Edwards Campus of the University of Kansas. He is also a teaching professor at the University, where he provides direct patient services, clinical supervision and interdisciplinary consultation. Alex has strong experience conducting independent medical exams, writing medical opinions, and performing case reviews and has over 10 years of clinical experience.
Prior to receiving his Doctorate from the University of Kansas, he graduated Summa Cum Laude from William Jewell College in Liberty, MO. Dr. Williams is a licensed psychologist in Missouri and Kansas.
In his role, Dr. Williams works closely with Dane Street’s Operational and Clinical teams, as well as with our Disability physician panel specialists to positively impact the quality of our reports and overall program metrics.
Dr. Dave T. Berndt is a Group Health Internal Medicine Specialist who provides medical oversight of our Group Health Review Program.
Dave Berndt, MD is a board certified General Internal medical specialist. He served his residency for Internal Medicine from 1998-2001 at the University of Wisconsin Hospital and Clinics and University of Nevada, Reno after receiving his Medical Degree at The Medical College of Wisconsin in 1998. He also served in an additional residency in Preventive Medicine as Chief Resident 2003-2005 at the University of Colorado Health and Science Center. There, he obtained his Masters of Science with a focus in Epidemiology and Biostatistics.
During 15 years of clinical service working in direct patient care, he also chaired the Quality Improvement Committees from 2011-2015 at Montana State University Student Health Center and from 2015-2016 at Colorado State University Health Network. In this role, he developed and refined clinic-wide EMR based population management systems for depression and other chronic disease states.
Prior to Dane Street, Dr. Berndt served as a Regional Medical Director from 2016-2021 for Aetna-CVS Health, implementing Utilization Management for pre-certification, concurrent review, retroactive review, and appeals. While at Aetna-CVS he also worked with hospital and provider groups on the construction and implementation of Accountable Care Organizations (ACO’s).
He resides in Nevada and is actively licensed in California, Colorado, Montana and Nevada. He currently works with Dane Street’s clinical and FL Operations teams for our Group Health division.
Dr. Ilyse Rosenberg is a physician in active practice and board certified in Psychiatry. She holds a B.S. from NYU and a Masters in Library Science from CUNY Queens College. She received her medical degree from the University of Medicine and Dentistry of New Jersey/Rowan University.
While attending college, she received Magna Cum Laude Phi Beta Kappa, Dean’s Honors Program, and Dean’s scholarship to Africa and China. She worked at Memorial Sloan Kettering Cancer Center in patient education. She completed her internship and residency in Psychiatry at Beth Israel Medical Center in New York City. While in residency she was an honored Reviewer at The American Journal of Psychiatry and General Hospital Psychiatry. She received the Research Fellowship at The Family Center for Bipolar Disorder and the Cynthia Horner Research Award at Beth Israel Medical Center in NYC. She has published case reports in the American Journal of Psychiatry, General Hospital Psychiatry, and Clinical Geriatrics.
She maintains clear and active licenses in the states of Washington, Arizona, New York, and Minnesota and is domiciled in Washington. She has presented posters at The American Association of Geriatric Psychiatry, American Psychiatric Association Annual Meeting, and the Eighth Annual International Conference on Bipolar Disorder. She has clinical expertise with child and adolescent psychiatry, and psycho-oncology, autism and bipolar disorder.
